Clients are required to enter and maintain an electronic form of payment on file and the payment method will be run after each session via the IVY Pay system.
​
A no-show/late cancellation fee of $55 will be assessed for missed appointments or appointments cancelled less than 24 hours before the time of the scheduled appointment. No-show and late cancellations are not billed tp, or reimbursable by insurance, as such, they are billed directly to the client.

*At present, Principle Therapy & Wellness bills the California and Michigan insurance plans listed above, only. However; clients may choose to submit documentation to their insurance provider for reimbursement of up front payments. Invoices and superbills can be provided upon request for documentation.

* If utilizing commercial insurance, it is the client's responsibility to learn and understand the out-of-pocket costs associated with services prior to receiving services. Co-pays, deductibles and co-insurance may be part of the client's responsibility to pay in conjunction with any payment covered by the insurance plan. Principle Therapy & Wellness cannot guarantee that your insurance provider will reimburse for treatment provided by out-of-network providers. If your insurance plan is not listed, Principle Therapy & Wellness providers are out-of-network for your plan. If you are unsure, Principle Therapy & Wellness can help you determine if your benefits will cover treatment. However, the full payment of services is the responsibility of the client if insurance will not pay.

Additionally, If coordination of benefit forms, other similar or different forms are necessary in order for your insurance provider to reimburse for services, it is the responsibility of the client to ensure that such forms are submitted once it becomes evident such forms are necessary. Failure to complete and submit necessary forms or other necessary steps that are the responsibility or duty of the client to facilitate insurance reimbursement, will result in out of pocket costs to cover payment for services rendered.
